For the purchase of a pair of Phonak Audeo Paradise 30-R (Rechargeable)  They come from my hearing center in Mesa, AZ.  I am licensed and board-certified, having working in this industry helping people hear better for nearly 20 yrs.  As the owner, I take pride in giving the highest quality service and products to my patients.
The process is:
Once you purchase a set of hearing aids, I ask that you provide me with a current (within 6 months) audiogram (hearing test)
I will personally review your audiogram, ask any questions regarding previous usage and preferences in programming and personalize the set to you.
Once they are ready, they will be carefully packed for shipment.
Your shipment will include:
2 hearing aids (receivers included)
combi-charger (desk charger that includes dehumidifier)
Domes to suit your hearing loss and ear size
Cerushield wax protection
Purchase agreement outlining the 3 year warranty and 3 yrs of loss and damage coverage
Packages are sent USPS Priority signature required and insured.  They come from my office in Mesa, AZ (Heartfelt Hearing Center)

“According to the FDA, it is in your best health interest to obtain a professional medical evaluation before purchasing a hearing aid and you are encouraged to do so before bidding on this item. I am required to provide to you a copy of the User Instruction Brochure for a hearing aid, review the contents of that brochure with you, and afford you time to read the brochure. You must provide to me either a signed statement from a physician or a signed waiver before I will deliver this item to you. Upon your request, I will also provide to you a copy of the User Instructional Brochure for the hearing aid or the name and address of the manufacturer or distributor from whom a User Instructional Brochure for the hearing aid can be obtained."
